Parent Resource Navigation Support

The Parent Resource Navigator Support is designed to assist families and caregivers in understanding and accessing public benefits, community services, and transition-related supports for individuals with complex communication, developmental, and adaptive needs.

The Parent Resource Navigator will provide guidance, information, and connection to resources in the following areas:
* Identification of community-based funding sources and financial assistance programs
* Support for families in rural or underserved areas with limited access to coordinated services
* Referral to independent IEP-related advocacy and support resources
* Assistance with applications for Medicaid Waivers
* Guidance related to Supplemental Security Income (SSI) applications
* Identification of community resources for individuals over age 21, including adult day programs, social participation opportunities, and peer connection programs
* Assistance with transition planning for individuals exiting bridge or school-based programs
* Identification and referrals to a list of Program Approved Service Agencies (PASA) and other service providers
Assistance exploring employment, volunteer, post-secondary, or adult day program options
* Resource identification for families relocating to or from another state, including comparable services and benefit structures

This program does not provide legal advice, clinical services, or direct funding. We help connect the dots for families to gain informational and navigation support in order to make informed decisions and better understand existing systems.


The Parent Resource Navigator Program is intended to:
* Reduce barriers to accessing services and supports
Improve continuity of care across medical, educational, and community settings
* Increase family confidence in navigating complex systems
* Strengthen coordination among providers, suppliers, and community agencies
* Promote informed, individualized decision-making
